The Park County Department of Human Services is currently requesting information from eligible individuals interested in performing duties on behalf of the Park County Collaborative Management Program. Below is a non-inclusive description of many duties that the Department and Program will require:

Coordinator, Collaborative Management Program (House Bill 1451)

REPORTS TO: the Park County Department of Human Services Director to support the Park County HB1451. Direct supervision is provided by the Park County Human Services Director in coordination with the HB1451 Executive Committee.

CONTRACT SUMMARY:
  • Average of 10 hours time required per week.
  • Schedule, facilitate, and document family and/or and support team meetings.
  • Accept reports regarding requests for services and/or other issues regarding adolescents and their families from phone calls, walk-ins, police reports, letters, and by other communication. Provide information to the public on programs and how they are accessed.
  • Assure that the mission, intent, objectives and purposes of the Park County Collaborative Management Program are effectively carried out according to the current Memorandum of Understanding.
  • Develop and maintain positive relationships with Collaborative Management Program partners and other community professionals through effective coordination, communication, and continuity.
  • Provide on-going feedback to partners on the implementation, progress, and sustainability of the Program.
  • Develop and effectively implement Collaborative Management Program policies and procedures as directed by the MOU.
  • Coordinate data collection, analysis, evaluation and reporting to the Collaborative Management Program partners. This includes tracking system development and ongoing reporting of aggregate data as they relate to outcomes.
  • Attend trainings and implement practices that support a family-directed care plan.
  • Act as liaison with local and state agencies and committees concerning public policy, programs, appropriations, standards, and other matters related to the Program.
  • Maintain confidentiality related to sensitive information and data.

REQUIRED SKILLS AND KNOWLEDGE:
  • A Bachelor’s degree or greater in a relevant field and practical experience is preferred.
  • Be knowledgeable of public services delivery and coordination.
  • Proficiency in Word, Excel, Power Point, and other relevant databases.
  • Be Knowledgeable of mountain area resources and referral opportunities.
  • Ability to work with diverse individuals and agencies.
  • Excellent written and oral communication skills.
  • Proficient in fiscal management related to tracking and billing hours worked.
  • Able to balance multiple priorities, various goals, and needs.
