Lost, Stolen, or Destroyed Permits
  • Within 3 business days after your permit is lost, stolen, or destroyed, you shall notify the issuing sheriff of the permit loss, theft or destruction
  • Failure to notify the sheriff is a class 1 petty offense
  • If a permit is lost, stolen, or destroyed, the permit is automatically invalid
  • The permittee may obtain a duplicate or substitute upon payment of $15 to the issuing sheriff and upon submission of a notarized Permit Lost or Destroyed Form, to the issuing sheriff that the permit has been lost, stolen or destroyed.
